Every summer, emergency departments treat thousands of older adults for heat-related illnesses. While high temperatures are the obvious culprit, many people don’t realize that some common prescription medications can quietly increase the risk of dehydration, dizziness, heat exhaustion, or even heat stroke. Adults over 60 are especially vulnerable because the body’s ability to regulate temperature naturally declines with age, and many older adults take several medications every day.

That doesn’t mean you should stop taking your prescriptions, but it does mean a five-minute conversation with your pharmacist before the hottest part of the summer could help prevent a medical emergency. Knowing which questions to ask is one of the easiest ways to stay safer during extreme heat. Here’s a look at what you need to be asking your pharmacist about your meds.

Some Medications Can Affect How Your Body Handles Heat

Many people are surprised to learn that certain medications can make it harder for the body to cool itself. According to the Centers for Disease Control and Prevention (CDC), some prescription and over-the-counter medications may reduce sweating, increase dehydration, affect blood pressure, or interfere with normal temperature regulation, raising the risk of heat-related illness.

Instead of guessing whether your prescriptions fall into these categories, ask your pharmacist whether any of your medications require extra precautions during hot weather. This is especially important if you take multiple medications because the combined effects can sometimes increase heat-related risks more than a single medication alone.

Which Medications Deserve Extra Attention?

Diuretics (“water pills”)
ACE inhibitors
ARBs
Beta blockers
Anticholinergic medications
Antihistamines
Some antidepressants
Antipsychotics
Stimulants

These medications don’t automatically become unsafe during hot weather. However, some may increase dehydration, reduce sweating, lower blood pressure, or make it harder for your body to cool itself. That’s why the CDC recommends reviewing medications before and during heat season rather than making changes on your own.

Ask Whether Your Hydration Needs Change During Hot Weather

Staying hydrated is one of the most common recommendations during periods of extreme heat, but the answer is not always as simple as drinking more water. Some adults over 60 have heart failure, kidney disease, or other medical conditions that require fluid restrictions, while others take diuretics or medications that affect electrolyte balance.

For example, someone taking a diuretic for high blood pressure may lose fluids more quickly during a heat wave than someone taking the same medication in cooler weather. On the other hand, people with heart failure or kidney disease may actually need to avoid drinking excessive amounts of water. Your pharmacist can explain whether your medications change your hydration needs and whether you should contact your prescribing clinician before making any adjustments.

The CDC advises older adults who have been told to limit fluids or who take “water pills” to ask their healthcare provider how much they should drink during hot weather rather than making changes on their own. Your pharmacist can help explain whether your medications increase the importance of monitoring hydration and whether you should discuss individualized fluid guidance with your prescribing clinician.

Medication Storage Can Matter More Than You Think

Heat doesn’t only affect people; it can also affect medications themselves. Some medications lose effectiveness if they are exposed to excessive heat, while others require refrigeration or careful storage to maintain their stability. Patients should ask about proper medication storage during hot weather and have a plan for medications that require refrigeration in case of power outages. A pharmacist can explain whether your prescriptions should never be left in a parked vehicle, carried in direct sunlight, or stored in a hot mailbox after delivery.

Some important things to keep in mind when it comes to medication storage:

Never leave medications in a parked vehicle.
Ask what to do during a power outage.
Mail-order medications shouldn’t remain in a hot mailbox for hours.
Insulin and biologics require special planning.

Know Which Symptoms Should Prompt a Call for Help

Many symptoms of heat-related illness, such as dizziness, confusion, weakness, headache, or unusual fatigue, can overlap with medication side effects or other health conditions. That overlap can make it difficult to know whether someone is experiencing dehydration, heat exhaustion, or another medical issue. Asking your pharmacist which warning signs deserve immediate medical attention can help you respond more quickly if something feels wrong. Also consider asking:

Do any of my medications increase my heat risk?
Should I drink more (or less) water during hot weather?
Which symptoms should make me call my doctor?
How should I store my medications during a heat wave or power outage?
Should I avoid exercising or being outdoors at certain times after taking my medications?

Warning Signs You Should Never Ignore

If you are feeling any of the following symptoms, it’s crucial that you pay attention.

Confusion
Fainting
Severe dizziness
Rapid heartbeat
Hot, dry skin
Heavy sweating followed by no sweating
Persistent vomiting
Difficulty walking

Heat stroke is a medical emergency. Call 911 immediately if someone develops confusion, loses consciousness, or has a body temperature above 104°F.

A Five-Minute Pharmacy Conversation Could Prevent a Summer Emergency

Pharmacists do far more than fill prescriptions. They’re trained to identify medication interactions, explain how heat can affect drug safety, and help patients recognize early warning signs before they become emergencies. A quick medication review can answer questions about hydration, storage, heat sensitivity, and when it’s appropriate to contact your healthcare provider.
