6 GLP-1 Risks That Hit Harder After 65 — Just As Medicare Starts Paying

Starting July 1, Medicare began covering weight-loss drugs like Wegovy and Zepbound for a $50 monthly copay. For seniors who’d been staring down price tags north of $1,000, that’s a real breakthrough.

But here’s what most of the headlines skipped.

These drugs don’t behave the same way in a 70-year-old body as they do in a 40-year-old one. The upside is real. So is the downside — and after 65, that downside gets heavier.

As Dr. Rozalina McCoy, an endocrinologist at the University of Maryland School of Medicine, told The New York Times, the risk-benefit calculus for these drugs “is very different for older adults than for younger people” — because the potential for harm is greater.

I’m not here to talk you out of anything. These medications are remarkable for the right person. I’m here to make sure you walk into your doctor’s office knowing exactly what to ask.

Here are six things worth weighing before you fill that prescription.

1. You could lose muscle right when you can least afford to

Your body starts shedding muscle in your 30s, and the losses speed up after 60. That’s just aging.

Now add a GLP-1. The weight you lose on these drugs isn’t all fat. According to the American Diabetes Association, lean body mass can account for 15% to 40% of the total weight lost.

For a 40-year-old, that’s a nuisance. For a 75-year-old, muscle is what gets you out of a chair and up the stairs — and keeps you out of a nursing home.

The fix isn’t to skip the drug. It’s to protect your muscles while you’re on it — resistance training and enough protein, worked out with your doctor.

2. The side effects can leave you dangerously dehydrated

Nausea, vomiting, diarrhea. They’re common with GLP-1s, and a Yale School of Medicine review found older adults tend to get hit with these gut side effects more often than younger patients.

Here’s the part that matters. Those symptoms drain fluids fast, and dehydration in an older body brings dizziness, weakness, and falls.

Yale’s Dr. Alissa Chen flags exactly this: a higher fall risk in seniors on these drugs, driven by dehydration.

A fall at 40 is a bruise. A fall at 78 can be a broken hip and everything that follows.

3. Fast weight loss isn’t always the win it sounds like

We’ve been trained to cheer any drop on the scale. In older adults, it’s more complicated.

Rapid, significant weight loss later in life is linked to worse health outcomes, not better ones. Geriatricians interviewed by the AAMC noted an association between losing more than 10% of body weight and higher all-cause mortality, especially in older men.

4. Your bones may be more fragile than the drug ads suggest

This one’s still being sorted out, so I’ll give it to you straight: The evidence is mixed.

But one 2026 study in the Journal of Clinical Endocrinology & Metabolism is worth knowing about. It followed more than 46,000 adults 65 and older with Type 2 diabetes. GLP-1 users had roughly an 11% higher risk of fragility fractures than those on other diabetes drugs.

Other research has found neutral or even protective effects on bone. So this isn’t settled science. It’s a question to raise with your doctor, especially if osteoporosis already runs in your health history.

5. They can scramble how your other medications work

GLP-1s work partly by slowing down your digestion. Food — and drugs — move through you more slowly.

Now picture the average 70-year-old’s medicine cabinet. Blood pressure pills, a statin, maybe a blood thinner, a thyroid tablet. Slower stomach emptying can change how those get absorbed.

Yale’s researchers warn that in older adults, these interactions can snowball into serious downstream effects. Bring your complete medication list to that first appointment. Every pill, every supplement.

6. The weight comes back when you stop — and this deal is temporary

Read the fine print on Medicare’s program: It’s a time-limited demonstration, currently set to run through the end of 2027. It’s a bridge, not a permanent benefit.

That matters, because when people stop taking GLP-1s, the weight tends to return. One analysis of eight trials found participants regained about 21 pounds within a year of stopping.

Here’s the cruel twist. You often lose fat and muscle on the way down, then regain mostly fat on the way back up. That leaves older bodies weaker than when they started. Don’t begin without an exit plan.

The bottom line

None of this means these drugs are bad. For the right older adult, under the right supervision, they can be life-changing.

But “over 65” isn’t a footnote on the label. It changes the whole equation.

Yale endocrinologist Dr. Kasia Lipska said it plainly: “We still don’t have clear evidence on how they perform in people over 65.”

So don’t let a $50 copay rush the decision. Bring the questions in this article to a doctor who knows your history — and make the call together.
