8 Surprising Things Medicare Does Not Cover

Many seniors assume Medicare will cover most of their healthcare needs, but that’s not always the case. In reality, there are several important services and everyday health expenses that Medicare simply doesn’t pay for. These gaps can lead to unexpected out-of-pocket costs if you’re not prepared.  Around 66% of retirees report around $375 in monthly healthcare costs. That can be a lot of money to come up with on a fixed income. That said, going into it with the right information can make all the difference. Here are eight things you might be surprised to find out that Medicare doesn’t cover.

1. Routine Dental Care and Dentures

One of the biggest Medicare coverage gaps is dental care. Original Medicare does not cover routine cleanings, fillings, extractions, or dentures in most cases. This often surprises seniors, especially since dental health is closely linked to overall health. The only exceptions are rare situations where dental care is medically necessary for another covered procedure. Without supplemental coverage, you’ll likely pay 100% out of pocket. Due to this, it is wise for seniors to look into Medicare Advantage or standalone dental plans.

2. Vision Care Like Glasses and Eye Exams

If you need glasses or a routine eye exam, Medicare usually won’t help. Original Medicare does not cover vision exams for prescriptions or corrective lenses. There are limited exceptions, such as after cataract surgery, but routine care is excluded. That means seniors often pay entirely out of pocket for eyewear and checkups. Considering how common vision changes are with age, this can become a recurring expense. It’s one of the most overlooked Medicare coverage gaps.

3. Hearing Aids and Hearing Exams

Hearing loss affects millions of older adults, yet Medicare does not cover hearing aids or exams to fit them. This can be frustrating since hearing aids can cost thousands of dollars. While some Medicare Advantage plans offer partial coverage, Original Medicare does not. Many seniors delay treatment because of the cost, which can impact their quality of life. Addressing this Medicare coverage gap often requires additional insurance or savings.

4. Long-Term Care and Custodial Care

One of the most expensive gaps in Medicare is long-term care. Medicare generally does not cover custodial care, such as help with bathing, dressing, or daily living activities. While it may cover short-term skilled nursing care after a hospital stay, ongoing assistance is not included. This type of care can cost thousands per month, especially in nursing homes. Many families are caught off guard by this limitation. Planning ahead is critical when it comes to this major Medicare coverage gap.

5. Routine Physical Exams

You might think an annual physical is covered, but that’s not exactly the case. Medicare does not cover traditional routine physical exams. Instead, it offers a yearly wellness visit, which focuses on preventive planning rather than a full physical. This difference can be confusing for many seniors. If you want a comprehensive exam, you may need to pay out of pocket. It’s another example of how Medicare coverage gaps can catch people off guard.

6. Cosmetic Procedures

Cosmetic procedures are almost never covered by Medicare. This includes things like facelifts, Botox, or other aesthetic treatments. The only exception is when a procedure is medically necessary, such as reconstructive surgery after an injury. Otherwise, you’re responsible for the full cost. While this may seem obvious, some procedures fall into a gray area and lead to confusion.

7. Care Outside the United States

Planning to travel abroad? Medicare generally does not cover healthcare services outside the U.S. This means any emergency care overseas could come with a hefty price tag. Some Medigap or Medicare Advantage plans may offer limited international coverage, but Original Medicare does not. For frequent travelers, this is an important gap to address. Travel insurance is often recommended to fill this Medicare coverage gap.

8. Prescription Drugs Without Part D

Many people are surprised to learn that Original Medicare doesn’t automatically cover prescription drugs. You need a separate Part D plan or a Medicare Advantage plan with drug coverage. Without it, you’ll pay full price for medications, which can quickly add up. This is one of the most important Medicare coverage gaps to address early. Choosing the right drug plan can significantly reduce your costs. Skipping it could lead to major financial strain over time.

Know What’s Missing Before You Need It

There are options for seniors who need to cover these gaps. Medicare Advantage plans often include benefits like dental, vision, and hearing coverage. Medigap policies can help cover out-of-pocket costs like deductibles and coinsurance. You can also purchase standalone plans for dental, vision, or long-term care. But you’ll want to be sure to compare prices so you don’t overpay.

Medicare is a powerful program, but it’s not all-inclusive. Knowing what it doesn’t cover is just as important as knowing what it does. These eight Medicare coverage gaps highlight where seniors are most vulnerable to unexpected expenses. The best thing you can do is to find something to fill the gap and plan ahead.
