5 Common Medications Linked to a Higher Risk of Bone Loss

Fractures and breaks are very common in seniors, affecting about 50% of women and 30% of men over age 50. The most common breaks impact the hip, wrist, and spine. Oftentimes, strong bones are something many people take for granted until a fracture or osteoporosis diagnosis changes everything. While aging, menopause, and family history all play major roles in bone health, some prescription medications can quietly contribute to bone loss over time. Knowing which drugs may affect your bones allows you to have informed conversations with your healthcare provider and take steps to reduce your risk.

Before we get started, it’s important to note that you should never stop taking a prescription medication without talking to your healthcare provider. Many of the medications listed below treat serious conditions such as asthma, epilepsy, depression, autoimmune diseases, or cancer. The goal is to understand potential risks, not to discontinue treatment on your own.

All of that being said, here are five common medication categories that research has linked to an increased risk of bone loss and fractures.

1. Corticosteroids Are the Most Well-Known Cause of Medication-Related Bone Loss

According to the National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS), glucocorticoids are one of the medications most strongly associated with osteoporosis. Long-term corticosteroids such as prednisone, dexamethasone, and methylprednisolone are among the leading medication-related causes of osteoporosis.

These drugs reduce the body’s ability to build new bone while increasing bone breakdown and decreasing calcium absorption, especially when taken for several months or longer. They’re commonly prescribed for asthma, rheumatoid arthritis, lupus, inflammatory bowel disease, and other autoimmune conditions, making them essential medications for many patients.

2. Proton Pump Inhibitors May Affect Calcium Absorption

Proton pump inhibitors (PPIs) such as omeprazole (Prilosec), esomeprazole (Nexium), and pantoprazole (Protonix) are widely used to treat acid reflux and stomach ulcers. While these medications are effective, long-term use has been associated with reduced calcium absorption, which may contribute to lower bone density and a higher fracture risk in some people.

The association between proton pump inhibitors and fractures appears strongest in people taking higher doses or using them for extended periods. Occasional short-term treatment for heartburn generally carries a much lower concern than years of continuous use, so it’s important to discuss the expected duration of therapy with your healthcare provider.

3. Some Antidepressants Have Been Associated With Lower Bone Density

Selective serotonin reuptake inhibitors (SSRIs), including sertraline (Zoloft), fluoxetine (Prozac), and escitalopram (Lexapro), are commonly prescribed for depression and anxiety disorders. Researchers have found that long-term SSRI use may be associated with lower bone mineral density and a greater risk of fractures, although the exact biological mechanism is still being studied.

Mental health treatment remains critically important, so patients should never stop taking antidepressants without medical guidance. Instead, individuals taking SSRIs for many years may benefit from discussing bone health, exercise, calcium intake, and bone density screening with their healthcare provider.

4. Certain Anti-Seizure Medications Can Weaken Bones Over Time

The National Institute on Aging and NIAMS both list antiepileptic medications among drugs that can contribute to osteoporosis risk. Several anti-seizure medications, including phenytoin (Dilantin), carbamazepine (Tegretol), phenobarbital, and some newer anticonvulsants, have been linked to reduced bone density.

These medications may interfere with vitamin D metabolism, making it more difficult for the body to absorb calcium effectively. Because epilepsy often requires lifelong treatment, healthcare providers may recommend periodic bone density testing, calcium and vitamin D monitoring, or lifestyle measures to protect bone health. Fortunately, many patients can reduce their fracture risk through proactive monitoring rather than changing medications.

5. Certain Hormone-Based Cancer Treatments Can Accelerate Bone Loss

Some hormone therapies used to treat breast and prostate cancer intentionally reduce estrogen or testosterone levels because those hormones can fuel certain cancers. Unfortunately, those same hormones also help maintain healthy bones, meaning medications such as aromatase inhibitors and androgen-deprivation therapies may accelerate bone loss during treatment.

Oncologists are well aware of this risk and often recommend bone density monitoring, weight-bearing exercise, and medications specifically designed to protect bone health when appropriate. Patients should never avoid these cancer treatments because of bone concerns, but understanding the risk helps ensure bone health remains part of the overall treatment plan.

Who Should Be Especially Aware of Medication-Related Bone Loss?

Not everyone taking these medications will experience bone loss. In many cases, risk depends on factors such as dosage, how long the medication is used, age, existing bone density, and other medical conditions.

The effects of these medications are often greater in people who already have other osteoporosis risk factors, including:

Women after menopauseAdults over age 65People with a family history of osteoporosisSmokersPeople with low vitamin D or calcium intakeIndividuals who have already experienced a fracture after age 50Anyone taking several bone-affecting medications at the same time

Having one of these risk factors doesn’t mean you’ll develop osteoporosis, but it does make regular conversations with your healthcare provider more important.

Questions to Ask Before Starting Long-Term Treatment

If you’re prescribed one of these medications for months or years, consider asking:

Will I need a baseline bone density (DXA) scan?Should I take calcium or vitamin D?Are there lifestyle changes that could help protect my bones?How often should my bone health be monitored?Are there alternative medications if my fracture risk becomes too high?

Should You Have a Bone Density Test?

People taking medications that may affect bone health for an extended period, especially older adults and those with additional osteoporosis risk factors, may benefit from discussing a DXA (DEXA) bone density scan with their healthcare provider. This painless test measures bone mineral density and can identify bone loss before a fracture occurs. Your provider can determine whether screening is appropriate based on your age, medical history, medication use, and overall fracture risk.

Protecting Your Bones While Taking Important Medications

The presence of a medication on this list doesn’t mean you should stop taking it or that bone loss is inevitable. In many cases, the benefits of treating conditions like asthma, depression, epilepsy, acid reflux, or cancer far outweigh the potential effects on bone health. Even if you are taking these medications, there are things you can do to prevent bone loss, including:

Getting enough calcium and vitamin DExercising regularlyAvoiding smokingLimiting excessive alcohol

If you’ve been taking one of these medications for years, it may be worth scheduling a medication review during your next appointment. When it comes to osteoporosis prevention, awareness and routine monitoring are often just as important as the medications themselves.
